Qualified Social Worker - Hospital Discharge Team - Worcestershire Service Care Solutions are currently recruiting for Qualified Social Workers in Worcester to work within the Hospital Discharge Team. This is a great opportunity for Adult's Social Workers to join a supportive team based within a hospital setting for Worcestershire County Council. The main responsibilities for this role are to conduct Mental Capacity Assessments, and to judge risk in order to support safe and timely discharge. -To be flexible and adaptable to the needs of the hospitals service. -To work with health colleagues, within a busy A & E department. -To record clearly on Framework & health records. This is initially a 3-6 month contract with a high chance of extension for the right candidate. Paying up to £25 ltd per hour!
